Reached via the persistent Make a referral button, not a navigation page. Included here so this document holds everything needed for review in one place. Structure carried over from the discovery responses, updated for the wording and selection-logic changes agreed with Jo and Jane.

General support, parenting programme, or select all that apply / unsure. Wording changed from "education programme" to "parenting programme".
2Who is making the referral
Self-referral, whānau referral or professional referral.
3Client details
Collected once, shared across the rest of the form.
4Safe contact and key questions
Contact preferences, safety questions, consent.
5Support or programme selection
Multi-select checkboxes plus Unsure, replacing the single Both option. CAPS triages from the selections made.
6Reason for referral and preferred outcomes
Brief description, concerns, risks, preferred outcomes.
